This post will be give information about the report keeping and documentation done in each components that is ICTC, STI, PPTCT, ART CENTERS, and Blood banks, reporting and documentation is very important in every department and in every organisation.
so as in STI clinics all registers are maintain in STI clinics like Master Registers, counselling diary, Stock registers, Referral Booklets, and many more Registers are maintained in STI clinics, by this the Quality of each components are seen how the staff are working and what they are doing? in their workplace and timely monitoring team goes there for checking and monthly and yearly review meeting are conducted so that the centers will work smoothly and the goal/aims of the NACP will be achieved.

Who should fill this reports
This reporting formats should be filled by all the STI/RTI Service providers and send to the corresponding reporting authority by the 5th of next month. the STI/RTI providers includes :
Providers at all designated STI/RTI and Obstetrics and Gynaecologic clinics(sentinels sites like areas/district hospitals, teaching hospitals attached to medical colleges etc)
Target Interventions providing STI/RTI services for High Risk Behaviour Groups.
